Position Details
Position: Registered Nurse (RN) or Licensed Practical Nurse (LPN) MDS Coordinator
Location: Near Pipestone, Minnesota
Start Date: Within 2 weeks of offer
Schedule: Monday–Friday | Day Shift
Guaranteed Hours: 40 Hours Per Week
Floor Responsibilities: Assist with direct patient care and support the nursing floor as needed.

What We're Looking For
We're seeking an organized and clinically strong RN | LPN who can lead the MDS process while ensuring compliance with state and federal regulations. The ideal candidate is detail-oriented, collaborative, and committed to delivering exceptional resident care.
Key Responsibilities
Coordinate and complete accurate MDS assessments in accordance with CMS guidelines.
Develop and update individualized resident care plans.
Ensure timely MDS submissions and reimbursement accuracy.
Collaborate with physicians, therapy, nursing, and interdisciplinary teams.
Monitor regulatory compliance and prepare for state surveys.
Audit documentation to ensure accuracy and completeness.
Educate staff regarding documentation requirements and quality standards.
Track quality measures and identify opportunities for process improvement.
Provide nursing floor support as needed to maintain quality resident care.
Participate in resident assessments, care conferences, and discharge planning.
Qualifications
Active South Dakota RN License or LPN License at time of Application
Previous MDS Coordinator experience in a Skilled Nursing or Long-Term Care setting
Strong understanding of CMS regulations, Medicare, Medicaid, and reimbursement processes
Excellent assessment, documentation, and communication skills
Ability to prioritize multiple responsibilities while maintaining accuracy
Team-oriented with strong leadership abilities
