Requirements & Skills

  • Current and valid Registered Nurse (RN) license in the state of Wisconsin
  • Experience in home health and/or hospice nursing preferred
  • Strong clinical assessment and patient care skills
  • Excellent communication and interpersonal abilities
  • Ability to work independently with a high degree of professionalism
  • Valid driver’s license and reliable transportation to visit patient homes
  • Proficient in documenting patient care and maintaining accurate records

Key Responsibilities

  • Provide skilled nursing care to patients in their homes, focusing on comfort, symptom management, and quality of life
  • Conduct thorough patient assessments and develop individualized care plans
  • Collaborate with interdisciplinary healthcare teams to coordinate patient care
  • Educate patients and families on disease management, medications, and care procedures
  • Monitor patient progress and adjust nursing interventions accordingly
  • Ensure compliance with all regulatory and agency policies
  • Maintain accurate and timely documentation in patient records
  • Support patients and families emotionally and provide hospice support during end-of-life care

Qualifications

  • Active Registered Nurse (RN) licensure in Wisconsin
  • Bachelor of Science in Nursing (BSN) preferred but not required
  • Prior experience in home health or hospice nursing highly desirable
  • Strong critical thinking and clinical judgment skills
  • Compassionate, patient-centered approach to care
  • Ability to manage time effectively and work independently
  • Reliable transportation for home visits

Compensation: Hourly rate $60.00 - $71.00, with paid weekly through Quick2Hire staffing agency.
