Requirements & Skills
- Current and valid Registered Nurse (RN) license in the state of Wisconsin
- Experience in home health and/or hospice nursing preferred
- Strong clinical assessment and patient care skills
- Excellent communication and interpersonal abilities
- Ability to work independently with a high degree of professionalism
- Valid driver’s license and reliable transportation to visit patient homes
- Proficient in documenting patient care and maintaining accurate records
Key Responsibilities
- Provide skilled nursing care to patients in their homes, focusing on comfort, symptom management, and quality of life
- Conduct thorough patient assessments and develop individualized care plans
- Collaborate with interdisciplinary healthcare teams to coordinate patient care
- Educate patients and families on disease management, medications, and care procedures
- Monitor patient progress and adjust nursing interventions accordingly
- Ensure compliance with all regulatory and agency policies
- Maintain accurate and timely documentation in patient records
- Support patients and families emotionally and provide hospice support during end-of-life care
Qualifications
- Active Registered Nurse (RN) licensure in Wisconsin
- Bachelor of Science in Nursing (BSN) preferred but not required
- Prior experience in home health or hospice nursing highly desirable
- Strong critical thinking and clinical judgment skills
- Compassionate, patient-centered approach to care
- Ability to manage time effectively and work independently
- Reliable transportation for home visits
Compensation: Hourly rate $60.00 - $71.00, with paid weekly through Quick2Hire staffing agency.
